1. Upon reciept of below documentation we will fax a contract to you. After we receive the completed and signed contract you will be approved to transport freight for ATS.
2. Copy of ICC Contract Authority
3. Copy of completed W-9 form. Click here for a blank W-9 Form or you can download one from the Federal Government Here. You will need Adobe Acrobat Reader to display and print these forms.
4. Cetificate of Insurance Naming ATS Logistics Services, Inc. as the certificate holder - with original certificate to follow. "WE MUST RECEIVE ORIGINAL CERTIFICATE VIA MAIL FOR PAYMENT AUTHORIZATION"
